Ms. Zhang underwent injection-based breast augmentation in Shanghai about 30 years ago using materials that were common at that time. After years of progression, she developed pain and swelling, with marked right-breast enlargement and granulomatous changes on MRI.
She sought care in several hospitals abroad without a definitive solution, then returned to China for treatment under time pressure.
After detailed assessment, Director Zhao confirmed the injected material and explained that surgery was feasible. MRI showed clustered granulomatous inflammation at the capsule base, requiring complete resection of inflamed tissue and capsule.
Given the patient's request to preserve postoperative aesthetics, Prof. Lan Mu was invited to co-operate, integrating breast surgery and plastic-reconstructive expertise for this complex procedure.
During surgery, both teams removed the injected material, excised inflamed tissue and capsules completely, and implanted directional breast expanders with appropriate saline filling. The operation lasted about three hours and the patient recovered steadily.
